Performance Rewards Program:
BCA is proud to reward day program and supported living staff who perform behavioral duties
with exceptional diligence and accuracy. Special tickets are routinely distributed to qualifying
staff when BCA observes exceptional performance. Once per month a drawing is held, and the
holder of the winning ticket may claim a fifty-dollar ($50) reward.
The winning ticket number from the drawing held on May 1, 2007 is 617795; the deadline for
claiming the reward is June 30, 2007.
The winning ticket number from the drawing held on June 1, 2007 is 617836; the deadline for
claiming the reward is July 31, 2007.
By submitting a claim for the reward, the claimant agrees to the following terms and conditions:
TERMS AND CONDITIONS
(1) A printout of this entire webpage must be submitted to BCA along with the winning ticket.
The Claim Submission Form at the bottom of the webpage must be legibly completed in full,
and the original ticket must be presented in good legible condition with no alterations, or the
claim shall be deemed invalid.
(2) The required items must be mailed together in a single envelope to BCA via the US Postal
Service and must be postmarked on or before the posted deadline, or the claim shall be
deemed invalid. BCA is not responsible for lost or misdirected mail, and therefore strongly
suggests use of a delivery tracking service.
(3) BCA's sole determination regarding the validity of the claim shall be final and binding.
(4) A fraudulent claim shall be subject to criminal prosecution under federal mail fraud statutes.
(5) A valid claim shall be paid within sixty (60) days, and shall be mailed to the claimant's
address as specified on the Claim Submission Form.
(6) Payment of the claim shall not create an employment, contractual, or other special
relationship between BCA and the claimant.
(7) The claimant shall be responsible for all taxes and tax reporting requirements associated
with the payment of the claim.
(8) The clamant agrees to unconditionally release BCA from any and all liability that may result
from the processing and/or payment of the claim, and to fully indemnify BCA for any and all
expenses that may be incurred in the event that any dispute arises from the claim.
(9) The claimant agrees that BCA may use, without limitation or further compensation, any and
all of the claimant's basic identifying information (i.e., name, city, state, place of employment,
job title, photo and/or other likeness) and a description of the reward amount for marketing
and/or other business purposes, and understands that this information may be presented
and/or published in a variety of mediums and formats (e.g., internet, television, radio,
newspapers, magazines, brochures, flyers, etc.)
(10) This promotion is void where prohibited by law.
(11) These terms and conditions are severable. In the event that any part is or becomes
unlawful or unenforceable, that part shall be removed and shall not invalidate the remainder.
